Please Note – HOME COLLECTION FOR INCAPCITATED PATIENTS

We are pleased to inform that for all incapacitated patients residing in Bangalore and those residing in the neighbouring states namely, Chennai, Hyderabad and Kochi, we have made an arrangement with M/s Lal Path Labs from the 5th October 2023 onwards for home collection of the blood sample, which they will to courier to us and we will forward it to CSIR­ IGIB, New Delhi.

The patients residing in the places mentioned above, may please contact Ms Lal Path Labs Call Centre No. 72048-46020 (Mr. Chandragupta) and schedule their visit for sample collection. The sample collector will provide you the following:

• .Patient information Brochure, which you can retain.
• .Informed Consent Form, which you need to read, sign and hand over.
• .Genetic Investigation in MND/ ALS, which you can retain.

For their services, M/s Lal Path Labs will be collecting Rs.300/- (Rupees Three hundred only) from you for drawing the sample and couriering to us using proper cold pack along with the copies of your medical records. Please note that the genetic testing report will take about 3 months and we will have it sent to you.

Secondly, please keep copies of all your previous medical records and investigations ready to hand over them to the person who comes to collect the sample. Your previous records will enable us to have subsequent online free consultation, evaluation and counselling with you or your caregiver.
